杭州某社区签约后Ⅱ型糖尿病病人健康管理效果研究

摘要目的验证签约对提升糖尿病健康管理效果的积极意义，从而为糖尿病规范化管理提供发展思路。方法通过回顾性研究，选取杭州市某社区2015-2016年间签约糖尿病病人240人，非签约糖尿病人60人，分析比较签约组和非签约组在糖尿病知信行，自我管理以及临床数据的差异。结果在糖尿病知信行上，签约组糖尿病并发症知晓率、饮食运动计划制定率、月就诊次数均高于非签约组（P﹤0。05）；签约组饮食控制，足部护理，限酒，遵医嘱服药四项糖尿病自我管理行为均优于非签约组（P﹤0。05）；签约服务提供一年后，签约组空腹血糖，糖化血红蛋白水平均小于非签约组（P﹤0。05）。结论签约服务对促进患者加强自我管理,提升社区糖尿病管理的服务能力具有重要意义,现阶段每名糖尿病病人和全科医生应该“应签尽签”，并通过健全管理手段来提升服务品质。88912

Abstract Objective To verify the positive significance of signing contracts in promoting the health management of diabetes mellitus, so as to provide the development ideas for standardized management of diabetes mellitus。 Methods In the retrospective study, contracted 240 diabetes patients, 60 non diabetes patients during 2015-2016 years in a community of Hangzhou, comparison group and non group signed contract of KAP in diabetes, self-management and difference of clinical data。 Results In diabetes KAP, signing group diabetic complications awareness, diet and exercise plan, monthly rate of visits were higher than those of non contract groups (P < 0。05); The four self management behavior such as diet control, foot care, limit alcohol, medication diabetes of Sign group are higher than those of non (P < 0。05); After signing the service for one year, the fasting blood glucose and glycosylated hemoglobin levels of the contracted group were smaller than that of the non signing group (P < 0。05)。 Conclusion The contracted services to promote the patients to strengthen self-management, is important to enhance the management of diabetes in community service ability, each stage of diabetic patients who should be signed to general practitioners should be signed, and through sound management means to improve service quality。源Q于D优G尔X论V文Y网wwW.yOueRw.com 原文+QQ75201`8766
