摘要问卷调查作为一种调查者了解特定社会群体对某一现象，某一事物的看法或意见的一种重要手段，有着非常庞大的使用主体和非常广泛的应用领域，对政府、企业、团体的决策有重要的辅助作用。而传统的问卷调查方式调查成本较高，回复率低，调查周期长，问卷分析也消耗很多人力。随着互联网的发展，个人电脑的普及，很多事情可以通过网上虚拟方式完成。本系统旨在通过PHP+MySQL的动态网页技术设计实现一个问卷调查系统网站，实现传统问卷方式完成的功能，弥补纸质问卷的不足，提高调查问卷的使用效率。19223

Title The Design And Development Of Campus Survey System
Abstract
Questionnaire survey as an important method for someone to find out people’s opinions and ideas in particular group on someting,has big main consuming body and a wide range of applications,as well plays an significant supporting role in decision-making of governments,enterprises and communities.But the traditional method of questionnaire survey costs much money and manpower,and has low response rate and long survey cycle.With the development of Internet and popularization of personal computer,many things can be done online in a virtual mode.This system aims to use active web page techonology PHP+MySQL to design and accompish a website of questionnaire survey which realizes functions of traditional way ,makes up for the shortage of paper quetionnaires and imroves efficiency in the use of the questinnaire.

1 引言
